Willard Arthur "Jack" Chapman, 67, of Mount Washington, KY passed away on June 27, 2014.
Jack was preceded in death by his parents, Arthur Chapman and Marie Bjorgo. He was also preceded in death by his sister, Shirley Colburn.
He is survived by his brothers, Thomas Chapman (Teresa) and Donnie Bjorgo; niece, Francis Colburn; and nephew, Paul Colburn.
Cremation was chosen following Jack's wishes. The family is planning a private memorial service at a later date.
